This is from a retired high school teacher of AP U.S. Government and Politics classes:

If you, as a higher education professional, are concerned about the quality of students arriving at your institution, you have a responsibility to step up and speak out. You need to inform those creating the policies about the damage they are doing to our young people, and how they are undermining those institutions in which you labor to make a difference in the minds and the lives of the young people you teach as well as in the fields in which you do your research.

The argument is that No Child Left Behind and other laws have created testing requirements in high schools that leave little room for students to develop critical thinking and writing. I have never gone to an American high school and I must admit I know frighteningly little about what goes on there. At Georgetown?s School of Foreign Service we select students based on their ability to write and think critically. Faculty actually read the essays of applicants. I have no complaints about the level of our freshmen but I know this is a very selective example.

So what should/can higher education professionals do?

Setting a password hint on administrator account in Windows Server 2008 R2

I've tried to change a password on administrator account using Control Panel\All Control Panel Items\User Accounts\Change Your Password but can't set a password hint. When I clicked What is a password hint I could read

"Because your computer is on a domain, you can't create a password hint."

Then I've opened computer properties and saw that the computer belong to workgroup only not a domain. Then I've run gpresult /v and it has printed a domain name which is a computer name and <Local Computer>. Could you explain it, I want to know if my computer belong to a domain or not as I don't remember if I configured PDC when installed the server or not? Also I don't have Active Directory Services role added to the server roles. And how to get information about domain controlles without adding ADS role?

(AP) ? The Pentagon is poised to extend health care, housing and other benefits to the same-sex spouses of military members by the end of August, but may reverse earlier plans to provide benefits to gay partners who are not married.

According to a draft Defense Department memo obtained Wednesday by The Associated Press, the department instead may provide up to 10 days of leave to military personnel in same-sex relationships so they can travel to states where they can marry legally.

While no final decisions have been made, the memo from Defense Secretary Chuck Hagel to top defense leaders would reverse an earlier plan that would allow the same-sex partners of military members to sign a declaration form in order to receive limited benefits, such as access to military stores and some health and welfare programs.

The recent Supreme Court decision extending federal benefits to legally married same-sex couples eliminates the need for such a plan, Hagel said in the draft.

"As the Supreme Court's ruling has made it possible for same-sex couples to marry and be afforded all benefits available to any military spouse and family, I have determined, consistent with the unanimous advice of the members of the Joint Chiefs of Staff, that the spousal and family benefits far outweigh the benefits that could be extended under a declaration system," Hagel wrote.

According to a U.S. official, the memo is under legal review by the Justice Department, and the Pentagon will not be able to take any action until that review is finished.

"Although we have bases and installations in all 50 states, not all state laws are equal when it comes to same sex marriage," a defense official said. "That is why we are looking at providing extra leave for same sex couples who want to get married to travel to a state where same sex marriages are legal." The officials were not authorized to discuss the memo publicly, so spoke on condition of anonymity.

Pentagon officials would not comment on the specifics of the memo. A Defense Department spokesman, Lt. Cmdr. Nate Christensen, said only that the Pentagon "is working alongside the Department of Justice to implement the court's decision as quickly as possible."

In February, then-Defense Secretary Leon Panetta announced that by no later than Oct. 1 the Pentagon would extend some limited benefits to same-sex partners of service members. Housing benefits were not included, but the plans called for same-sex partners to get special identification cards granting them access to commissaries and other services.

The benefits would be contingent on the service member and their same-sex partner signing a declaration that they were in a committed relationship.

At the time, officials said that if the Supreme Court ruled on the federal Defense of Marriage Act the issue would be revisited. The act prohibited the federal government from recognizing any marriage other than that between a man and a woman.

In late June, the court cleared the way for legally married gay couples to be recognized under federal law, and also allowed same-sex marriages in California to resume. It did not issue any sweeping declarations that would allow same-sex couples to marry anywhere in the country.

When the ruling was announced, Hagel said the Pentagon would reassess the department's decisions on benefits for same-sex couples, and also begin the process of extending benefits to same-sex spouses of military members.

In the new draft memo, Hagel says the department intends to treat all married military personnel the same and "make the same benefits available to all military spouses, regardless of sexual orientation."

But, recognizing that same-sex couples are only allowed to marry in a limited number of states, Hagel said the provision allowing service members to travel to states where the unions are legal is a way to help overcome those challenges.

Defense officials estimate there are 18,000 same-sex couples in the active-duty military, National Guard and Reserves. It's unclear how many of those are married.

The repeal of the ban on openly gay military service took effect in September 2011.

Obama hosts Hillary Clinton for lunch ? 'friendship' on the menu

President Barack Obama hosted Hillary Clinton for lunch on Monday, treating his former secretary of state to grilled chicken, pasta jambalaya and salad, amid speculation that her possible 2016 White House run could also be on the menu.

White House spokesman Josh Earnest told reporters that the purpose of the lunch was "chiefly social" and emphasized that the two seasoned political players have a "genuine friendship" and just wanted to "catch up."

"It's largely friendship that's on the agenda today, it's not a working lunch," said the spokesman, who denied that Obama was already looking at possible successors.

Clinton's successor at the State Department, former Massachusetts Sen. John Kerry, was seen entering the White House a little after she did. But "the table is being set for two," Earnest said. Kerry was to meet later with Obama.

Clinton and Obama were expected to discuss the newly relaunched Middle East peace talks, the spokesman said.

Obama?s press office had announced the luncheon meeting, but there was to be no media access.

An embarrassing scandal involving top Clinton aide Huma Abedin may be another reason the former secretary of state wasn't planning to speak to reporters. Abedin is married to Anthony Weiner, who is refusing to quit the New York City mayor's race even after acknowledging he had continued the sexting behavior that forced him out of Congress in 2011.

Even barely past the halfway mark in 2013, ?Hillary 2016? rumors have swirled around Washington ? notably because of the possibility that this could pit her against Vice President Joe Biden.

?I can die a happy man never having been president of the United States of America,? Biden recently told GQ. ?But it doesn?t mean I won?t run.?

The magazine described Biden as being ?a Hillary Clinton away from the White House.? That?s a nod to the loose inside-the-Beltway consensus that he won?t run in 2016 if she does.

Hillary stepped down as America?s top diplomat Feb. 1 after serving out Obama?s entire first term. She has been giving speeches around the country, some of them handsomely paid, others for free.

Rumors that she might be looking to make Bill Clinton the first husband have stoked partisan political passions. There is a ?Ready For Hillary PAC? encouraging her to run and a ?Stop Hillary PAC? that, well, the name is a bit of a clue.

And NBC has decided to produce a four-hour miniseries based on Hillary Clinton's life from 1998 onward, starring Diane Lane in the lead role. That would likely start the story arc with the country in the grips of the scandal over Bill Clinton's affair with former White House intern Monica Lewinsky. It would then carry through her successful Senate race in New York, her defeat at Obama's hands in the fight for the Democratic presidential nomination in 2008, and finally her time as secretary of state.

LONDON: Twitter, the social media site, is under pressure in Britain to make it easier for Internet users to report abuse after more than 30,000 people petitioned it over the case of a feminist campaigner who says she was repeatedly threatened with rape.

Caroline Criado-Perez helped lobby the Bank of England to make a woman, 19th century novelist Jane Austen, the new face on the country's 10 pound note, to defuse criticism that women were under-represented on the currency.

She was then "targeted repeatedly with rape threats" by ill-wishers objecting to her activity, according to an online petition, which called on Twitter to urgently add a 'report abuse' button to its service.

Some users proposed a one-day boycott of Twitter to protest against what they said was its failure to address the issue.

Twitter's General Manager for the UK Tony Wang promised to suspend all accounts found to be in breach of its rules.

"We take online abuse seriously," he wrote, saying the company was testing ways to simplify reporting of abuse.

(AP) ? A series of explosions rocked a central Florida propane gas plant and sent "boom after boom after boom" through the neighborhood around it. Eight people were injured, with at least four in critical condition.

John Herrell of the Lake County Sheriff's Office said early Tuesday there were no fatalities despite massive blasts that ripped through the Blue Rhino propane plant late Monday night. Officials initially scrambled to find more than a dozen employees after the explosions.

"Management is comfortable saying all of those they knew were there tonight have been accounted for," he said.

Tavares Fire Chief Richard Keith said possible causes of the explosion may be either equipment malfunction or possibly human error. Sabotage was not suspected.

One person injured in the explosion was listed in critical condition at University of Florida Health Shands Hospital and three others were listed in critical condition at Orlando Regional Medical Center. Herrell said some others drove themselves to area hospitals.

Tavares Battalion Commander Eric Wages said five workers walked up to a command center firefighters set up near the plant Monday night with skin hanging off their arms, torso and faces. He said their arms were outstretched and they were in complete shock.

The Blue Rhino plant, which is northwest of Orlando, refilled propane tanks typically used for barbecues and other uses. There were some 53,000 20-gallon tanks at the plant on Monday.

Herrell said a crew of 24 to 26 people was working the overnight shift when the explosions occurred around 10:30 p.m. Monday.

Tuesday morning, smoke still billowed from a storage container on the property, which consists of a couple of warehouses next to each other. The parking lot was littered with thousands of blackened 20-gallon propane containers.

Nearby, three 33,000-pound tanks of propane sat untouched. Lake County Battalion Chief Chris Croughwell said the hoses designed to spray water on the large tanks in case of fire, did not go off as planned because they had to be manually activated. "Most sane people don't stick around for an event like this," he added.

Tavares Mayor Robert Wolfe said Tuesday that he was surprised to learn the hoses at the plant had to be manually activated. If Blue Rhino reopens the plant, Wolfe said he plans to raise the safety issue. "That way, it's fail safe," Wolfe said. "We're lucky those tanks didn't explode."

Croughwell said firefighters who responded to the initial fire had to wait to enter plant site because conditions were so dangerous. Just as they were finally about to go in, four tractor trailers parked next to the large propane tanks caught fire.

If the large tanks exploded, Croughwell said, "it would have wiped us out."

Video footage on WESH-TV in Orlando showed fires burning through trucks used to transport propane tanks, which were parked at the plant. The fire sent plumes of smoke into the air hours after the blast.

Keith said the explosions shook his house several miles from the plant. "It truly sounded like a car hit our house," he said.

Herrell said about 50 homes were evacuated Monday night but residents were allowed back in about four hours later.

Marni Whitehead, 33, who lives less than a mile from the plant, said she was in bed ready to go to sleep when she heard a loud boom.

She ran outside and saw other neighbors outside and then they saw the explosions.

"We knew right away it was the plant, the propane plant," Whitehead said. "After that, it was just sort of panic."

Whitehead likened the explosions to Fourth of July fireworks. "And it was just boom after boom after boom," she said.

Herrell said officials believe the fire was contained and wouldn't spread to another part of the plant.

According to the Leesburg Daily Commercial, the plant was built in 2004 and employs fewer than 50 people.

Archbishop of Canterbury 'furious' at Church of England's stake in payday lender Wonga.com

The Archbishop of Canterbury is said to be furious after an attempt to take on Britain?s biggest payday lender, Wonga.com, has exposed the church?s own ethical shortcomings.

Under Archbishop Justin Welby?s lead, the Church of England plans to support credit unions as a kinder alternative to money-lenders who offer instant cash loans at unholy rates of interest.

Archbishop Welby told Total Politics magazine this week he was proud the church was ??putting our money where our mouth is??.

??I?ve met the head of Wonga and ... I said to him quite bluntly we?re not in the business of trying to legislate you out of existence, we?re trying to compete you out of existence,?? he said.

The church would encourage church members to volunteer at credit unions, and invited local and independent lenders to use church buildings. It also launched a new credit union for clergy and church staff, charging its members low rates of interest to borrow money.

However an investigation by the Financial Times newspaper found that the Church of England?s own pension fund has links to Wonga.

The fund invested? ?1 million? ($1.7 million) in Accel Partners, which led a 2009 venture capital fundraising round for Wonga.

The ?5 billion pension fund has an ethical investment policy that is supposed to specifically exclude payday lenders.

In a statement, the archbishop?s office at Lambeth Palace said it was grateful to the Financial Times for pointing out this ??serious inconsistency of which we were unaware??.

It had launched an internal investigation and would review the holding in Accel and ??investigate whether there are any other inconsistencies??.

The BBC reported that the archbishop was furious at the oversight.

A former oil industry executive, he has written extensively on the relationship between finance, morality and religion.

He also sits on the UK Parliamentary Commission on banking standards.

In his college dissertation he argued that corporations are capable of sin, and in interviews last year before becoming head of the worldwide Anglican communion, he spoke of his concerns about the power of the financial world, telling The Guardian that ??the opportunity to make enormous sums of money is enormously corrupting??.

The ?2 billion payday lending industry has ??deep rooted?? problems, according to Britain?s Competition Commission, which recently launched a major 12-month investigation.

David Fisher, director of consumer credit at the Office of Fair Trading, told London?s Daily Telegraph they ??found problems everywhere, from the way they advertise the loans through to the way they collect the debt??.

Some lenders had been encouraging customers to extend loans rather than pay them off, leaving them further mired in debt.

The archbishop?s challenge to the industry had been welcomed by British Chancellor George Osborne, who said he was? looking at better regulation of the industry as well as a bar on advertising high-interest loans.

Wonga responded with a tongue-in-cheek advert promoting its ??10 commitments?? to responsible lending.

Wonga.com was founded in 2006 and quickly became the country?s leading payday loan company with its streamlined, web-based technology (it was the first to create an instant lending app for the iPhone) and its huge advertising spend.

The company offers loans up to ?1000? ($1660) for 31 days. It charges 365 per cent interest per annum ? one per cent a day ? but after fees, the representative rate of interest displayed on the website is 5853 per cent.

Errol Damelin, Wonga?s founder, said this was often cheaper than getting a bank overdraft.

Wonga.com describes its customers as tech-savvy young professionals with a temporary need to cover a cash shortfall. More than 90 per cent of its 1 million customers are in paid employment.

Baseball: ThunderBolts light up for 5-2 win over Gateway Grizzlies

After scoring just once in the first two games of their series vs. the Gateway Grizzlies, the Windy City ThunderBolts were in dire need of some offensive production Wednesday.

The T-Bolts mixed that sense of desperation with a heavy dose of aggression. They stole four bases in the first three innings, while stretching singles into doubles and doubles into triples to put up five runs in the first four innings.

Combine that with another terrific start from Dyllon Nuernberg and Windy City salvaged the final game of the series with a 5-2 victory at Standard Bank Stadium in Crestwood.

?We?ve talked a lot about what we want our team to be,? the T-Bolts? Andrew Brauer said. ?We want our identity to be scrappers. We want to bunt, take extra bases and do anything we can to manufacture runs.?

The fans in attendance for ?Splash Day? were not only treated to a win from Windy City (24-34), but also were witness to a surprise appearance by the Stanley Cup.

After an event at the adjacent Caesar Park, the Cup was brought to Standard Bank Stadium and paraded around the field after the game?s final out.

That was a thrill for Brauer, a Mokena resident.

?It was very cool,? he said. ?I followed (the Blackhawks) when they won it last time (in 2010) but never got a chance to see the Cup. Getting to see it here was a moment I?ll never forget.?

The T-Bolts broke through in a big way early, scoring three times in the first. After Mike Torres walked and stole second, Ryan Soares drove him in with a double down the left-field line. Chad Cregar singled in Soares.

Cregar stole second and took third on an errant throw before scoring on Zach Aakhus? sacrifice fly.

?We?ve been struggling to produce runs ... we?re going to do whatever it takes to score,? Soares said. ?We were aggressive and we knew if we could get Dyllon some early runs, we?d be in good shape.?

Nuernberg (6-1) was unusually wild, walking six, but he held the hard-hitting Grizzlies (35-22) in check, limiting them to one run on two hits in seven innings. He struck out nine and lowered his ERA to 2.33.

?My command wasn?t there like usual, but once I got in the stretch I was like, ?I?m going to throw strikes, and they?re going to have to get three straight singles off me to score,? ? he said. ?There?s nothing like going out for the second up 3-0 already. We had all the momentum.?

Wednesday was (kind of) a sad day for Taylor Swift fans. Boston's KISS FM canceled its "Taylor Swift's Biggest Fan" contest today after it was revealed that a 39-year-old man named Charles won the grand title.

The radio station updated its contest page with a statement clarifying the reasoning for their decision.

"Disappointingly, we have determined that the integrity of the 'Taylor Swift's Biggest Fan' contest has been compromised,' it read.

"In accordance with our contest rules, effective immediately, the contest has been terminated. We apologize to all of our loyal listeners who have participated."

Weeks ago, Charles shot up to the No. 1 spot after his pic ended up on numerous websites asking for its anonymous users to help him meet the "22" singer.

One post asked people to vote for Charles and help "crush the dreams of these girls and give him a chance to make a complete ass of himself by blatantly just sniffing her hair with cameras rolling."

The grand prize was slated to be two free tickets to one of her concerts and a backstage meet-and-greet with the 23-year-old.

But while one fan's dreams may have been crushed, another got the surprise of her life.

An eyewitness tells E! News that Swift went out to dinner with Ed Sheeran and Austin Mahone at Ralph's Italian Restaurant in Philadelphia, Penn., last Friday and they took photos with the head chef.

"I called my mom to tell her Swift was there, and she wanted me to take a picture because she is a big fan," he said.

"My son has autism, and while I'm at work my mom and my son love to listen to her music. He loves her music!"

The blond beauty then shocked the chef by handing over two tickets to her concert, which was the following night.

"It was so kind-hearted of her," the chef gushed. "My son is 11. It was my son and I's first concert we've ever been to! It was really special."

We now know that Microsoft?s Surface RT tablet will go down as one of the biggest consumer electronics bombs of the past year, perhaps second only to the Facebook-centric HTC First smartphone. But does this mean Microsoft is doomed to irrelevancy? Of course not. And as long as the company keeps raking in cash from Office and Windows, it will have the resources it needs to be competitive in the mobile space.

Canon previews a handheld version of its MREAL mixed reality tech at SIGGRAPH, we go handson video

As a complement to its MREAL Mixed Reality headset, Canon is showing off a handheld version of the technology this week at SIGGRAPH. The new version functions much like the head-mounted one, enabling the use of markers or (as was the case here) sensors to render images in real space. Something you'll want to keep in mind: this is still an enterprised-focused device. That said, it doesn't make the tech demo and usage scenarios any less cool to gawk at. The demonstration we saw here in Anaheim involved a Kabuki dancer out in the center of the demo area. Details like shadows and wrinkles in the performer's clothes were rendered in real time -- just as if a real person were performing. A collection of sensors mounted around the top of the demo stage allowed us to look around the space while the projected image reacted to our position. Not once did we lose sight of the action.

Two other demos for the head-mounted display (that can easily translate to the handheld unit as well) caught our attention, too. First, a boat motor was projected in real space using augmented reality markers, allowing the user to inspect a scale model of the engine for training or other purposes. The ability to deconstruct the engine and see how different portions of it worked was all available to the user. Next, we saw a set of markers wrapping a rectangle projected a model of a Canon DSLR housing. Both of these scenarios offer a more in-depth look at 3D models before the prototyping phase or any steps are taken towards production. A quick look at the Kabuki demo and our best in-dance commentary awaits on the other side of the break.

SNP cabinet visit: housing debt and constitution top of SIC?s agenda

SIC convener Malcolm Bell says the local authority hopes to use this week?s visit of senior Scottish Government figures to make the case for its historic housing debt to be dropped ? and for the isles to be handed more autonomy.

SNP ministers are due to arrive at Sumburgh on Wednesday ahead of its monthly cabinet meeting, to be held at Mareel on Thursday.

The council recently teamed up with its counterparts in Orkney and the Western Isles to launch ?Our Islands, Our Future?. The document is designed to seek further powers for the three island groups regardless of which way next year?s referendum on Scottish independence goes.

Mr Bell said he was delighted Shetland had been included in the cabinet?s summer itinerary. Visiting politicians will include First Minister Alex Salmond, finance secretary John Swinney and education minister Mike Russell.

?It provides a valuable opportunity for the council, and indeed the wider community, to liaise with the First Minister and his cabinet secretaries,? Mr Bell said.

?As well as promoting what?s good about Shetland, we plan to use the visit to raise a number of issues including Shetland?s historic housing debt, homelessness and constitutional future. I have specifically asked for a meeting with the First Minister over the housing debt.?

This newspaper launched a campaign in late May aimed at persuading the UK and Scottish governments to take action on the ?40 million housing debt. If nothing is done, council house tenants face a rent rise of 10 per cent or more from April 2014. ?

A petition directed at both governments has garnered 2,233 signatures to date. It is aimed at Holyrood housing minister Margaret Burgess and Westminster chief secretary to the Treasury Danny Alexander.

Mr Bell said: ?Both governments are trying to pass the buck on this saying it is an issue for the other government. What we are saying is ?no, you both have a locus in it?, and we have to come to a joint solution.

?I feel we have a very strong and unique case to have this issue dealt with once and for all, so that we can avoid having to raise council house rents to unacceptably high levels and build desperately needed new housing.?

It is understood Scottish civil servants have grown ?very concerned? about the housing debt issue after the new council brought it back onto the political agenda.

This newspaper has been seeking, without success, an interview with Mr Alexander ? who campaigned to have Highland Council?s housing debt dropped before taking office ? for the past two months.

In late June a Treasury spokeswoman acknowledged that Mr Alexander had met an SIC delegation at Downing Street in December to ?better understand their concerns?. But he believes it is down to Holyrood and Lerwick Town Hall to sort it out.

The spokeswoman said: ?He hopes that the Scottish Government and Shetland council can find an appropriate solution.?

Contrary to Mr Alexander?s stance, several councillors ? including political leader Gary Robinson ? are clear that it is primarily the UK government?s responsibility because the debt was incurred in the 1970s and 1980s when housing was a Westminster matter.

Responding to the petition last month, Scottish housing minister Margaret Burgess said Shetland was ?fortunate to have access to financial reserves and I?d encourage the council to continue to review how these could play a part in their financial plans.?

Ms Burgess did say she looked forward to meeting with the SIC ?over the coming months? to discuss the situation. She is due to visit the isles in early August.

Shetland MSP Tavish Scott said he wanted Mr Salmond to ?respond positively? on two key issues ? addressing the housing debt and ending ?centralisation?.

He called for ?commitment after commitment? from the SNP administration ? not ?photo opportunity after photo opportunity?.

?The first action should be to cancel the housing debt hitting Shetland?s housing tenants and the second is to stop the removal of power from island councils which is resulting in the centralisation of public services,? Mr Scott said.

?Baltasound was used by the oil industry for offshore flights in the past,? he said. ?Now with so much expansion I am asking that Chevron fully explore that potential again.??

Unusually for Psychological Medicine, we publish below six letters concerning the paper by White et al (2013) on the PACE Trial. The UK Office of the Journal received 15 letters criticising aspects of this paper, but it seemed unlikely that all of these letters originated entirely independently since a number arrived on consecutive days and reiterated the same points. Nevertheless, in the spirit of scientific openness, we have published six of the letters which cover the main criticisms, and invited Professor White to reply to them.

In their paper on recovery rates in the PACE trial, White et al. (2013) acknowledge that `objective measures of physical activity have been found previously to correlate poorly with self-reported out- comes. Yet, there is no attempt to utilize the Six Minute Walking Test results. The best results were a mean of 379 metres walked in the graded exercise therapy condition, a gain of 67 metres in 52 weeks, 35 metres more than the specialist medical care (SMC)-only group (White et al. 2011). The cognitive behaviour therapy group showed no improvement compared with the SMC group. The distance of 379 metres is exceeded by patients listed for lung transplantation (Kadikar et al. 1997) and by older patients with chronic heart failure (Lipkin at al. I986). Given the recognized problem with self-reported outcomes, reliance solely on such measures leaves open the question of the validity of the recovery criteria of PACE.

White and colleagues conclude from the results of the PACE trial that ?recovery from CFS (chronic fatigue syndrome) is possible, and that CBT (cognitive behavioural therapy) and GET (graded exercise therapy) are the therapies most likely to lead to recovery? (White et al. 2013).

However, in the body of the text, they qualify their use of the term ?recovery?. Citing Nisenbaum at al. (2003) they write, `recovery may be taken to imply that the patient has made a transition from ill health to remission and also is at little risk of recurrence? but then acknowledge that, in the absence of longitudi- nal data, it is not possible to discriminate between remission and recovery in CFS.

Thus, in the current paper, ?recovery? does not mean recovery as understood by Nisenbaum but `recovery from the current episode of the illness?, a state described by Nisenbaum as `remission?.

This difference is important because CFS is known to pursue ?a fluctuating course with periods of relative remission and relapse? (CFS/ME Working Group. 2002) and Cochrane reviews of CBT (Price et al. 2008) and GET (Edmonds et al. 2004) have reported inconsistent findings at long-term follow-up, with some studies showing that initial gains can diminish with time. Writing about the PACE trial, Edmonds et al. concluded `Even when the results of that study are available, it is possible that uncertainty will remain. Further randomized studies are needed, with longer follow-up, to determine whether patients who respond to exercise stay well or relapse.`

White et al. (2013) report various recovery rates from chronic fatigue syndrome (CFS) following the PACE Trial. However, additional information would have been useful.

White et al. use a selection of broad criteria to define recovery, none of which allow one to be confident recovery has been achieved. Firstly, Chalder Fatigue Questionnaire (CFQ) and SF-36 Physical functioning (PF) scores within the normal range are in fact possible at baseline. This means it is possible to have fatigue that is classed as `severe, disabling and affected physical and mental function? and yet satisfy this particular recovery criterion.

Secondly, not satisfying the Oxford criteria only requires a change on just one measure, and the change may be minimal, across a threshold, e.g. going from an SF-36 PF score of 65 to 70 or a CFQ (bimodal) score of 6 to 5. A sign that this criterion is not that stringent can be seen with the fact that 41% of the specialist medical care (SMC) group, which received no active treatment, no longer met the Oxford criteria at 12 months, much higher than recovery rates seen in previous studies (Cairns & Hotopf, 2005).

Finally, a CGI score of 2, which means a participant rated as `much better? but not `very much better? also gives no assurance that somebody had recovered. It seems quite possible that many with CGI scores of 2 have simply improved but not recovered.

It is debated whether cognitive behaviour therapy (CBT) or graded exercise therapy (GET) reliably facilitate recovery in chronic fatigue syndrome (CFS). As such, any data on this issue, such as those presented by White et al. (2013), are always of interest.

The trial was not blinded, however, with participants, therapists and research assessors aware of the treatment group for each individual (White et al. 2007). Consequently, there is the possibility of significant response bias. Indeed, while the CBT group performed better than the adaptive pacing therapy (APT) and the specialist medical care only (SMC) groups on the self-rated SF-36 physical functioning (SF-36 PF) scale, there were no significant differences and minimal numerical difference on the more objective six-minute walk distance test (6MWD) (White et al 2011).

This discrepancy between subjective and objective outcome measures is not a novel finding in the CFS literature. Wiborg et al. (2010) analysed three randomized control trials (RCTs) of three CBT interventions, finding that while fatigue was improved in the CBT groups compared to waiting-list controls, there was no difference in actometer readings between the two groups. Moreover, a mediation analysis showed changes in physical activity were not related to changes in fatigue. Similarly, in a GET RCT, Moss-Morris et al. (2005) found that an increase in physical fitness did not mediate the treatment effect of reduced fatigue. In an uncontrolled trial of a graded activity programme, Friedberg & Sohl (2009) reported improvements in SF-36 PF and fatigue while actometers showed overall reduction in total activity levels.

The 6MWD is one objective outcome measure White et al. (20l3) could have incorporated into their recovery criteria (White et al. 2007). Reference ranges for 6MWDs, which adjust for gender and age inter alia, exist for healthy adults (e.g. Chetta et al. 2006; Casanova et al. 2001). Then, after calculating the new recovery percentage with the 6MWDs, analyses could be preformed to compare the means with predicted values.

Important outcome data from the PACE trial (White et al. 2011) appears to be missing from the paper describing recovery in ME/CFS (White et al. 2013) and the participants do not appear to have been asked whether they had recovered as a result of receiving cognitive behaviour therapy (CBT), graded exercise therapy (GET) or Pacing.

The paper would have been improved had three speci?c markers of recovery been reported. First is the receipt of a state sickness or disability benefit. Claiming such a benefit indicates that the person is still ill and has not recovered. This data was included in the cost analysis study (McCrone et al. 2012) that reported: `Receipt of benefits due to illness or disability increased slightly from baseline
to follow-up.?

Second is employment or education status. The recovery paper argues that ?Return to work is not, however, an appropriate measure of recovery if the participant was not working before their illness and is in?uenced by other factors such as the job market.? However, a sustained return to meaningful paid employment, or education, or the ability to do so, is an objective marker of recovery.

Third is ability to mobilize. Recovery in a condition whose cardinal clinical features relate to mobility ? exercise-induced muscle fatigue and weakness ? must be matched with an ability to mobilize in a normal and timely manner. The overall results for all the treatments in the PACE trial relating to changes in the six-minute walking test from baseline to 52 weeks do not represent a return to normal levels of activity. It can be seen that the figure for all the treatment groups at 52 weeks are below the 402m reported to be present in patients with class 3 heart failure (Lipkin et al. 1986). So the results for those who had recovered ? who should now be achieving a much higher distance ? ought to have been included. In addition, the question could be raised as to how it is possible to meet the entry criteria for the PACE trial with a Short Form-36 physical function subscale score of 65 yet leave the trial as recovered with a lower score of 60.

The term `recovery? implies a sustained return to symptom-free health with the ability to repeatedly and reliably participate in all aspects of normal life ? employment, education, social activities. etc. Without this information it is difficult to conclude that these patients have in fact recovered.

The de?nition of recovery from any chronic illness is challenging. We therefore agree with Cox (20I3) and Courtney (2013) that no single threshold measurement is sufficient; this is why we measured several domains of improvement and combined them into a composite measure of recovery (White et al. 20l3). Shepherd (2013) suggests asking patients whether they recovered as a result of [our italics] receiving a treatment; we did not ask this since it is not possible for individuals to ascribe change to one particular source in exclusion from all others, such as regression to the mean or external factors.

Maryhew (2013) suggests self-ratings may be biased when participants cannot be masked to treatment allocation; this may be true, but is inconsistent with cognitive behaviour therapy (CBT) being more effective than adaptive pacing therapy (APT) when treatment expectations were signi?cantly lower before treatment (White et al. 2011).

We dispute that in the PACE trial the six-minute walking test offered a better and more `objective? measure of recovery, as suggested by Agardy (2013), Maryhew (2013), and Shepherd (2013). First, recovery from chronic fatigue syndrome (CFS), which is defined by a patient?s reported symptoms, is arguably best measured by multiple patient-reported outcome measures, rather than a single performance test. Second, and importantly, there were practical limitations to our conduct of the walking test. Due to concerns about patients with CFS coping with physical exertion, no encouragement was given to participants as they performed the test, by contrast to the way this test is usually applied (Guyatt et al. I984; American Thoracic Society, 2002). Rather than encouragement, we told participants, `You should walk continuously if possible, but can slow down or stop if you need to.? Furthermore we had only 10 metres of walking corridor space available, rather than the 30-50 metres of space used in other studies; this meant that participants had to stop and turn around more frequently (Guyatt et al. I984; Troosters et al. I999; American Thoracic Society, 2002), slowing them down and thereby vitiating comparison with other studies. Finally, we had follow-up data on 72% of participants for this test, which was less than for the self-report measures (White et al. 2011).

Economic data, such as sickness bene?ts and employment status, have already been published by McCrone et al. (2012). However, recovery from illness is a health status, not an economic one, and plenty of working people are unwell (Oortwijn et al. 2011), while well people do not necessarily work. Some of our participants were either past the age of retirement or were not in paid employment when they fell ill. In addition, follow-up at 6 months after the end of therapy may be too short a period to affect either benefits or employment. We therefore disagree with Shepherd (2013) that such outcomes constitute a usefulcomponent of recovery in the PACE trial.

We agree with Carter (2013) that there is a difference between sustained recovery and temporary remission; this is why we were careful to give a precise definition of recovery and to emphasize that it applied at one particular point only and to the current episode of illness (White et al. 2013).

Despite the complexities of measuring recovery, we believe that our approach of using multiple self-report measures provides a reasonable approach to inform clinicians? and patients` choice between available treatments.

The ?ndings from the PACE trial are clear; however we measured recovery, CBT and graded exercise therapy (GET) were more likely to lead to recovery, when added to specialist medical care (SMC), compared to either adding APT or SMC alone. Recovery after SMC alone, using our composite criteria, was only 7% ? the same as that without treatment (Cairns & Hotopf, 2005) ? whereas three times as many (22%) recovered after receiving CBT or GET. The PACE trial has shown that both CBT and GET are moderately effective, safe, cost-effective, and are more likely to lead to recovery (White et al. 2011, 2013; McCrone et al. 2012). These treatments should now be routinely offered to all those who may benefit from them (Crawley et al. 2013].

Former MVP Ryan Braun suspended for rest of season

NEW YORK (AP) ? Former National League MVP Ryan Braun has been suspended without pay for the rest of the season and the postseason in what appears to be the first of a series of suspensions resulting from baseball's investigation of a Florida clinic accused of distributing performance-enhancing drugs.

MLB Commissioner Bud Selig announced the penalty Monday, citing Braun for multiple "violations" of baseball's drug program and labor contract. The Milwaukee Brewers star will miss at least 65 games.
